Provides preventive health services that will assist mothers, children and families in
obtaining their optimal health status. this includes:
- increasing access to prenatal and infant care
- educating women of childbearing age about health and nutrition during pregnancy
- identifying and addressing potential health problems early
Services are provided through care coordination that emphasizes a strong case
management component. Case management involves developing family care plans,
referring, coordination and following up with patients to assure access to services.
