VITAL STATISTICS
The Leon County Health Department serves as the county's vital records office for
birth and death certificates. The Vital Statistics Office is located at 2965
Municipal Way on the 1st floor (map).
The Health Department can issue birth certificates for the years 1930 to the
present for births within the State of Florida. All birth certificates for
years not mentioned above must be requested from the State Office of Vital Statistics in
Jacksonville, Florida.
Death certificates are issued by the Health
Department for the years 2006 to the present for those individuals who passed away in Leon County.
Death certificates for other years or counties must be requested from Jacksonville.
Birth certificates are issued by the Health Department
for years 1917 to the present for births in Florida. Birth certificates can be issued to parents, to an individual 18 years of age or older
and , with proper documentation, to legal guardians. Death certificates can be
issued to the family of the deceased or to a licensed funeral director.

Birth certificates issued by the Health Department cost $12.00. Death
certificates issued by the Health Department cost $10.00 each. For Leon County only
and years available are 2006 to the present.
